Woman With Multiple Disabilities Seeking A Government Housing Grant

I'm a 38-year-old woman with multiple disabilities seeking a government housing grant. I've always worked hard and I did the best I could for my two sons, ages 18 and 15.
Four years ago I became disabled. I have more than one heath condition.
Now, I receive a disability check each month that is barely enough to get us by. I live with my mother who has a mental condition that affects my children and myself drastically.
We have had to leave and sleep in our car. She cannot afford the monthly bill on her own. But, we cannot continue living in such an environment.
Three years ago I was married and I went out east with my husband to visit his family. We stayed gone about a month. When we returned everything we owned was gone. Now I have nothing.
I want to find a government housing grant for people with disabilities that will help me buy a trailer and land, plus the furniture and everything else that is needed for a home.
I'm a simple person. I'm not asking to live in a home that is above what I can afford. My children and I simply to live a peaceful life in a home where no one can tell us to leave.
