Vietnam Disabled Veteran Seeking Grant or Cosigner For Adapted Home

I am a 63-year-old Vietnam Vet male. I have been married to the love of my life, Marilyn for 45 years.
We have 2 beautiful daughters, Crystal and Heidi. We also have 7 wonderful grandkids.
I spent 12.5 years in the US Army from 1966-1979, during which time I spent a year in Bien Hoa Vietnam. I also did 3 tours in Germany as well as several places Stateside.
The disability I have is congestive heart failure. I have had mini heart attacks as well as a stroke.
Until I was falling down a lot and losing consciousness briefly, I didn't know I had a problem.
When I went to the hospital because I'd fallen and cut my forehead open I was put on monitors and was told I had a heart problem.
I was admitted and had many tests done to find the cause. After all the tests I was given a pacemaker.
Over the course of 2 years my heart problems have gotten progressively worse. I now require a walker and/or a cane to walk.
I have an adjustable bed so I can sleep elevated. As of 2010 I am 100% permanently disabled.
Our hardship concerns a home that I can get around in without problems with my walker.
Another issue is having enough space to be able to have my youngest daughter and her two smallest children living with us, so that she can help my wife and me with all our daily chores, meals, meds, etc.
I receive SSDI as well as my VA disability. My wife also receives SSDI. Other than that we have no other income.
I am looking for a grant to help us purchase a home we've seen that will be ideal for our situation.
It is an old group home that has been made into a duplex. It is also on 1.36 acres.
We have a small down payment, but the biggest problem is that due to circumstances our credit rating is to low for any mortgage.
We are working on getting it up, but the home is available now.
