Recent Computer Software Graduate Needs Government Transportation Grant Money

My name is Autumn Osborne. I am in a wheelchair. I graduated from college with an associate degree in computer software.
We have a van but my uncle has to lift me from my wheelchair into the van. We had a van with a lift, but it was old and the motor blew up. I am trying to get a job but it's almost impossible without transportation.
My mother and I live on social security and really can't afford to buy one. I know there is help out there but I cannot find it. We live in southwest Virginia in the mountains.
I cannot go anywhere without a strong man to be there to lift me. That is embarrassing at times since I am 22 years old.
I would love to go to work and help myself. If there is help out there please someone help me. A government grant for accessible transportation would be much appreciated.
