Polio Disabled Woman Seeks Government Grant For Van With Wheelchair Lift

My story starts long ago in 1949. I had Polio and have walked with a brace and two crutches ever since. It's made life an uphill struggle at best.
Always being very independent, accomplishing feats along the way, I worked 14 years and am now living on Disability. May I say nothing could be done without the Lord in my life.
Now I have a hip replacement because I fell and broke my hip in 2002. In the past year and a half, I have fallen three times.
I've broken my knee and my upper leg clear through to the bone. I have a rod and screws there.
Two months ago my wrist was shattered. I had surgery and they put in a plate and screws. It is very stiff and I'm right handed, so I'm having therapy.
I also have had two pelvic bones broken at different times, then a thumb and then a finger. So with much dismay the doctor now says I must stay in an electric wheelchair to avoid more broken bones.
This brings me to my need for a grant. I can't walk anymore so I can't get to the car. I drive, but can't get into and out of the van. I desperately need a van with a chair lift.
The reasons are obvious. I could get a part-time job and do the things that normal people do. But I cannot get out.
Well, I can't even imagine when I read this over, it sounds so simple, but to live it is another story like life has been taken away.
I've always been a very busy and full of life person. I love to help people and encourage them to go on and press on.
Please help me know what to do. I'm anxious to hear from anyone who can help in any way. Sincerely, Nancy.
