Physically And Emotionally Disabled Distressed Couple Hopes For Debt Relief

My boyfriend and I are both disabled and receiving social security as our only source of income. He has two disabilities: one mental and the other physical.
I have a mental/emotional disability. It seems that life in general has taken its toll on us and our money during the last year.
We owe money to many different places and things continue to pile up for us. I have to go to therapy once a week and now owe over $100 to the therapist's office.
Our electric bill was on a budget and we owe almost $500 on that so they have raised our monthly budget to over $100 a month. Our income is low and paying over $100 a month for a utility isn't something we can afford.
Our washer and dryer quit working on us and we live in the country so doing laundry at a Laundromat isn't an option so we had to rent to own. It's twice the price of what we would have to pay if we just bought a set from a store and it's another monthly payment to our already taxed income.
Furthermore, we have an older car we got a loan on a while back and the car is wearing out. We need to replace it but we still owe $3000 on it.
I had some identity theft and was unable to pay some loans I got in the past so my credit has now been ruined beyond repair.
My boyfriend didn't have any credit and was able to get two credit cards but we maxed them both out just buying things we need around the house. One is over $1,000 and the other is about $400.
Further still, all our furniture is old and in disrepair. It needs to be replaced and is becoming hazardous. Pieces of wood and nails are starting to stick up out of our sofa.
My boyfriend hasn't had new socks or pants or shoes for years and I have limited clothing that still fits. As I type now I'm in a dress with holes and shoes with holes.
Each person or family has individual needs and the needs of the disabled like us are vast. I don't know who or where to turn to or how to get help.
We want to return to a level where we aren't so indebted to others or to where we can have new clothes and furniture, and a car that works properly.
I come here seeking hope and help in these issues. Hopefully in a way I can understand since my disability interferes with that sometimes. Thank you for taking the time to read this.
