Housing Grant Or Equal Help For Parents Of Cerebral Palsy Disabled Boy

My name is Emanuel and I am 25 years old. I am currently engaged to the mother of my three wonderful children: Brianna 7, Arianna 4 and Emanuel Jr. 6.
Little Emanuel has a diagnosis of cerebral palsy and is wheelchair bound.
I was recently released from prison and my fiancée has struggled for two years supporting my children as well as myself during my incarceration.
We are currently on the verge of eviction. I have tried public assistance but to no avail. The landlord wants his money by December 9, 2011 or he threatens to throw us on the street. The amount is 7000 dollars.
I am so scared I don't know what to do. Someone please help.
My son was diagnosed with cerebral palsy at the age of six months and he is such a blessed child. He is six years old physically but mentally he is less than two years
My son is in constant need of wipes, diapers and certain equipment and medications.
We live a building that's not wheelchair accessible. It has no elevator and it is roach and rat infested.
I have been looking for work almost every day. With a felony it's so difficult but I'll keep on looking.
My son collects $460/month SSI.
I am in need of a grant that can help me pay the back rent. If I do this there is an organization willing to pay our rent. But the back rent issue must be resolved first for them to help us.
