Disabled 50 Year Old Biker Woman Seeks Government Disability Grant

Me when I was happy living life.
I am 50 years old with several medical disabilities. I am seeking grants for the disabled that match my unique disabilities.
The right grant would allow me to continue my independence as an individual who loves and enjoys life.
Because I am hard of hearing I qualify for grants for that disability and because I have vision problems I qualify for those types of grants.
I also have arthritis and neuropathy, both of which limit my activities during different weather situations.
I have urinary/bladder problems, resulting in incontinence. This too limits my activities.
I have COPD from smoking all my life so I have to use inhalers, as well as a breathing machine.
I love to drive myself to doctor appointments, the grocery store, and physical therapy, but don't have my own vehicle.
My last vehicle had transmission problems and ended up in the junkyard. I got that vehicle donated to me by the Christian Motorcyclists years and years ago.
I haven't driven since. I can't afford to because my SSI income is too low.
Being a motorcyclist was the only activity that kept me feeling free and relaxed. It kept my anxiety and panic attacks down because I was able to socialize at my own discretion according to my disability.
Now I am in my rented mobile home not socializing with church and friends because I can't get around.
The grant process is confusing to me. I can't seem to get help for a personal grant that I would not have to pay back to meet my needs for my disabilities.
I had a stroke years ago so sometimes I get confused as to what type of grant I should even apply for.
I have this dream that some day even with all my disabilities I could own my own farm or home in country. But that dream seems to fade as the years keep going by because I can't get help for a grant.
Where does one turn to for assistance to get these grants? You're probably saying they have loans/grants for wheelchair vehicles but not a custom trike.
Well, then I scream discrimination if that is the case, because my last motorcycle was customized for my disabilities thanks to other motorcyclists donating their money and time to customize it for me.
A farm or house would also have to be customized for me cause I can't walk long distances without stopping for long period of time to catch my breath due to my COPD.
If you know how I can obtain these disability grants or loans, please tell me what my next steps would be.
