Aging Disabled Couple Seeks Government Grant Money For Multiple Debt Relief

My wife and I are both on SSDI. My disability is due to a motorcycle accident 25 years ago, and my wife's is because of COPD and emphysema, which she was diagnosed with in June of this year.
As a result of my accident, I have lost one arm, had many surgeries, had my left knee reconstructed, leaving me with 50% use of it, had a pin placed in my right femur, had a bruised brain, which caused short and long term memory loss.
I am in severe pain 24 hours a day. I just recently suffered a heart attack and had a stint placed. I am on Medicare, which only pays a percentage of the bills, and I have zero extra money to pay the balance.
I suffer from depression due to my ability to provide. My wife was working up until last June when she was hospitalized and diagnosed with COPD and emphysema and, was not able to continue working.
We have thousands of dollars in medical and credit card bills that we are unable to pay. We were forced to move due to inability to pay the amount of rent they were asking.
We now live in a farmhouse that needs a lot of repairs and is on the market. We are barely able to pay for the basics and by the end of the month we usually have no food.
We are looking for grant money to help pay medical expenses, utilities, credit cards and our vehicle and daily living expenses.
We are also looking for help to buy the house we are living in as we are getting up in age and it is getting to hard to move, in fact, too expensive to move. We just don't have the money.
