61-Year-Old CMT-Disabled Woman Seeks Government Grant For Wheelchair Van

I'm 61 years old and have been handicapped all of my life. I've been wheelchair-bound with CMT (Charcot Marie Tooth) disease the past few years.
I can't go anywhere without my wheelchair, so I need financial assistance to get a wheelchair van.
But I can't afford one because I'm on a fixed income and the Muscular Dystrophy Association only helps getting a wheelchair.
I can't go places like shopping or to the doctor by myself. My husband helps me to my destinations but we are having problems with our current vehicle.
My husband quit his job in order to help me with my needs. He does all the work around my home like cleaning and cooking.
He also helps bathe me and place me on the toilet when I need to go to the bathroom.
He has also modified our home with bars in the bathroom and has built me a wheelchair ramp in case I do get out.
So if you can help me out by getting me a van it would help me go places. We only visit my in-laws once a year and they only live two hours away.
