55 Year Old Man Seeks Government Grant For Wheelchair Accessible Renovations

I am 55 years old. I have been on Medicare disability since 2001 due to degeneration of all my joints.
In the years of 2001, 2002 and 2003 respectively, I had both hips and left knee replaced.
Since 2005, there have been 8 surgeries on my right knee with the joint being replaced twice.
In 2009, I was in four hospitals for four months as a result of gall bladder surgery. I was on a respirator with a tracheotomy, chest tube, feeding tube and in a coma.
Thanks and Praise to my Holy God that I survived the illness. I have also been in the hospital twice already this year with pneumonia and MRSA in the right knee joint.
My wife and I live in a modest three-bedroom, two-bath home. I cannot walk long distances and have an electric chair that is heavy on my conventional home.
The cabinets in my home are not wheelchair accessible. Neither is the shower. We have been in our home for two years.
In those two years, the floor in the bathroom that I use has become weak from the heaviness of my wheelchair. So has the floor in front of the back door.
We need help with the repairs needed on our home to make the floor stable. We would appreciate it if you could point us in the direction of a government grant to do such work.
