44 Year Old Bipolar Disabled Man Seeks Grant Money For Disabled Van

I'm a single white male, 44 years old. I'm looking for grant money to get a disabled van.
I was diagnosed with bipolar disorder back in 1995. In 2004 I was prescribed lithium and in 7 months became toxic.
At the same time I went through a heat stroke, which affected my brain stem.
The effect put me in a wheelchair. I was unable to walk or speak. With speech therapy I was able to talk again.
I collect SSDI on a monthly basis. After paying rent and other bills I don't have enough left over for a van payment.
The only income that I collect is SSDI. I also get $16 in food stamps per month, and I go to the food bank once a month.
I am looking for a government transportation grant to help me purchase a disabled van.
